SmartCare

Fotolia_4129349_XSThe SmartCare system is at the core of the ADL suite of systems. The system has been developed with professional therapists over the last 8 years and uses proven state-of-the-art matching algorithms which were the first of their kind in the industry and which remain the most comprehensive and proven algorithms in their field. The SmartCare system can be used in the following ways:

SmartCare for monitoring supported self-assessment (SmartAssist)

As soon as a Service User begins a full supported self-assessment (Option A) in SmartAssist, the assessment can be viewed in the Regional Assessment page of SmartCare. As the supported self-assessment goes through the process of answering questions and viewing solutions matched to the Service User environment and personal capability, a member of the OT team can monitor all transactions as they happen through the SmartCare system. If needs be the Service User can save the assessment in SmartAssist and a member of the OT Team can continue the assessment through the SmartCare system.

If a supported self-assessment shows a Service Users ability is out of the scope of the system, the OT Team have a detailed referral for the Service User in SmartCare which usually includes the Service Users measurements e.g. bath width, length and depth. The Service User can send a message at the end of the supported self-assessment which is encrypted and shown through the Regional Assessments page of the SmartCare system.

Scenario One: A clinic assessment for a new Service User.

The Regional Administrator uses the measurement booklet facility within SmartCare to print off the required measurements for the problems experienced by the Service User. The measurements are sent to the Service User along with a clinic appointment and the measurements are completed with the help of a friend or relative. (We have found this to be an empowering experience for the Service User and most are happy to take control of their care and ask for help in completing the measurements.) The measurements are posted back to the Regional Administrator who 'finds' the Service User and enters the measurements for that Service User into the SmartCare system.

On the day of the clinic the advisor searches for the Service User and begins the assessment. As the Regional Administrator has already completed environmental measurements they are auto completed and the assessment moves along seamlessly. If local solutions are matched to the Service User the advisor can add the solutions to the Service User's basket. Once all problems have been assessed the basket is confirmed and the order is sent to the local stores immediately with an email notification of a new order. The stores team can then log into their version of SmartCare to view the orders within seconds with local requisition forms. As the SmartCare system is set to take a cautious approach no solutions may be matched. The advisor can show the assessment to the Occupational Therapist present and make an informed decision using equipment if available (e.g. the clinic is held in a equipment centre).

Scenario Two: A clinic assessment for a Service User who has completed a supported self-assessment in SmartAssist.

As the Service User has completed an assessment already and answered any measurement questions the Regional Administrator does not have to create a new user in SmartCare or send out any measurement booklets. A clinic appointment is sent to the Service User and on the clinic day the advisor can continue the self assessment the Service User started in SmartAssist, within SmartCare which saves re-entry and a quicker end result.

Although the scenarios above give you a guide on how SmartCare can work for you, the system can fit many models of working and each mobilisation is unique to your organisation.
